    Coursework Approval Form - Certified Staff

    Complete the linked form in PowerSchool Records.  You can find the Coursework Approval Form - Certified Staff under the Available Forms tab on the left hand side.  Please note that approval requests need to be submitted at least two weeks prior to the course start date.

    *Once the form has been submitted, you are able to confirm submission by selecting Files, Coursework Approval/Tuition Reimbursements.  You will receive the form back once approved.

  • Tuition Reimbursement Forms

     

    Tuition Reimbursement Form - All Staff

    Complete the linked form in PowerSchool Records.  You can find the Tuition Reimbursement Form under the Available Forms tab on the left hand side.  You will need to submit a copy of your grade report and a copy of your receipt for the paid tuition with the reimbursement form. Any questions, please email Lashanta Givens at lgivens@pennsburysd.org 

    *Once the form has been submitted, you are able to confirm submission by selecting Files, Coursework Approval/Tuition Reimbursements.  You will receive the form back once approved.

     

  • Column Movement/Salary Credit 

     

    Column Movement/Salary Credit Application

    Complete the linked form in PowerSchool Records.  You can find the Column Movement/Salary Credit Application under the Available Forms tab on the left hand side.  You will need to have all official transcripts necessary for movement sent electronically to Robin Peters at rpeters@pennsburysd.org.

    *Once the form has been submitted, you are able to confirm submission by selecting Files, Coursework Approval/Tuition Reimbursements.  You will receive the form back once approved.

  • Change of Name & Address Forms

  • Please send an email  dept_hr@pennsburysd.org to receive the forms needed to make a change to your name and/or address.  These forms will be sent through Powerschool Records.  Please have your updated social security card ready to upload for any name changes.
     
     
  • Leave Forms

     

    Leave of Absence Request Form - All Staff

    To request a medical, child rearing, FMLA, or personal leave, please complete the linked form in PowerSchool Records.  You can find the Leave of Absence Request Form under the Available Forms tab on the left hand side.

    *Once the form has been submitted, you are able to confirm submission by selecting Files, Leave of Absence.  You will receive the form back once approved.

     

    Professional Development Sabbatical Request Form - Certified Staff

    Complete the linked form in PowerSchool Records.  You can find the form Sabbatical - Professional Development under the Available Forms tab on the left hand side.

    Any person employed in the public school system of Pennsylvania who has completed ten (10) years of satisfactory service as a professional employee shall be eligible for a sabbatical leave of absence.

    The deadline for the Sabbatical Request Form is March 1st.

    *Please note that if you would like any of the coursework to be reimbursed, you would also want to submit a coursework approval form prior to the course start date in order to be eligible for reimbursement.

    *Once the form has been submitted, you are able to confirm submission by selecting Files, Leave of Absence.  You will receive the form back once approved.
