PRESCRIPTION INSURANCE INFORMATIONDENTAL INSURANCE INFORMATIONLIFE INSURANCEEach employee is provided with a term life insurance policy, at district expense, in the amount equal to 100% of the employee's annual base salary rounded to the nearest thousand dollars. Because you will be taxed on the premium value of your life insurance benefit over $50,000, you may wish to send Human Resources a signed letter requesting to cap your life insurance at $50,000.Beneficiary Form - When updating your beneficiary, please click on Beneficiary Form (located under forms). Send the completed form to Dawn Boraski in the Human Resources Department.LONG TERM DISABILITY INSURANCEYou may enroll in voluntary disability insurance. This is offered through Unum. There are several different options available (i.e.: 30 day, 60 day or 90 day waiting periods and a benefit of 60% or 66.67% of salary). Cost is based on the amount of your salary and calculated according to the option you choose. The premiums can be deducted through payroll. Unum has final approval of eligibility under this plan. They will send you a letter informing you of your eligibility and option. Click here for Long Term Disability Enrollment FormCAFETERIA SECTION 125 FLEXIBLE SPENDING ACCOUNTPennsbury does offer a way to save for out-of-pocket medical, prescription, dental, vision and daycare expenses on a pre-tax basis through a Section 125 Flexible Spending account. Enrollment is done once a year during the Open Enrollment Period. This program runs from July 1st through June 30th each year, with an extension allowing employees to continue to submit any claim with a date of service within that school year until September 15th. Cafeteria Section 125 Flexible Spending Account PamphletEMPLOYEE ASSISTANCE PROGRAMFor help and assistance in dealing with personal and workplace problems, the district has an employee assistance program operated by HealthAdvocate. They can help you in dealing with such issues as family and marital problems, emotional problems, stress and depression, anxiety, alcohol and drug dependency, etc. They provide trained and licensed counselors. Their services are totally confidential. The district never receives any information regarding the names of staff using HealthAdvocate services.
