What to do in case of a work-related injury:

1.    Report injury to supervisor.  

2.    Call SDIC 800-445-6965 or click here to report an injury and receive a claim number.  If you need to seek medical attention, please use one of the providers on this form.

3.    Email Robin Peters, Supervisor of Human Resources @rpeters@pennsburysd.org to report the injury.  

4.    Once reported, you will be receiving the workers' comp related forms to complete and upload into Powerschool Records.  The forms you will receive are also linked below.

5.   Give all Medical Centers the SDIC address to send any bills that you may incur for this injury.
